Lab work: urinalysis, drug screenings, Hemoglobin, blood pressure, blood draw (sent to Lab Corp & Quest Diagnostics).  Blood work is collected between 8:30 am and 9:30 am check hours of operation page for the schedule or call office a 850-697-3420.

FREE Diabetic Support Group: Monthly support group gathers to share diabetes tips for success and support each other. Call the office to be placed on our Diabetic e-mail and contact list. 

FREE Smoking Cessation Support: In partnership with the Health Department and the C.H.I.P. (Community Health Improvement Partners) look for workshop dates.

FREE Health Screenings and HealthFairs:
Occasional Fall and Spring Fairs offer FREE health screenings and information such as Hypertension, COPD, BMI (Body Mass Index), Diabetes

Referrals: Our nurse will manage your referrals to specialists 

Physicals: Sports physicals at a reduced rate, Work & school physicals conducted with next day appointment. Paper work on file in office.  DOT physicals performed by approved DOT physician.

ADHD: screenings, diagnosis, and monitoring of medication. in partnership with Franklin County School and ABC school.  

Sliding Scale Program: Our goal is to provide quality medical services regardless of an inability to afford full pricing.  Speak with our Administrative Assistant to get qualification information for our SPA (e.g. sliding scale and reduced fee) Program for the un/under insured. Patients must have proof of no income or insurance, lack of income via social services or social security, and current family expenses.
Complete the attached form and bring to the office.

Birthday Club: On your birthday, bring in your birthday postcard and receive a discount

Payment Plan: Weekly, Bi-monthly, Monthly arrangements can be made to pay fees. Speak with the office manager to make arrangements and sign payment plan.
